1)  COMPLETE THE APPLICATION FORM

The student is to personally fill in all data requested on the application form.  Answers should be complete, thorough and legible.

Below is the contract for student's parent/guardian:

*The Parent Contract is included in the Student Application PDF form only.  It is not included in the Student Application Online form.  It will need to be filled out separately if the students completes the online student application form.  Parent(s) can choose PDF or online version.

(Some forms require signatures which will have to be done on paper or digitally and sent through E-mail, Fax, or Mail.)

 


2)  READ STUDENT HANDBOOK AND SIGN APPLICATION

Both the student and a parent should read the student handbook and then sign and date the application form.

3)  ARRANGE FOR 3 STUDENT REFERENCES 

Student Reference Forms should not be sent directly from the student nor student's parent(s) along with their Student Application Form.  Reference Forms should come directly from the referrer.  Below are the two options available for submission.  The links for the reference forms may be emailed to the referrer.

Who should fill out the Student Reference Form?:

a) A principal or recent school teacher
b) Your local pastor
c) An adult friend who has known you for over a year


*If the PDF version of the student application is used, the student reference still has the option to choose either the online or PDF reference form.

 


4)  SUBMIT COPY OF TRANSCRIPT AND GRADES

An official primary school grade report and/or high school transcript along with the most recent report card should be submitted.  Home Schooled students must complete the “Home School Credit Sheet”.  See forms below.

 

  • ADDITIONAL INFORMATION FOR HOMESCHOOLERS

Ouachita Hills Academy welcomes applications from young people who have been properly home schooled. Standardized test scores are required for students lacking official transcripts from recognized educational programs. Home school classes and grades, when recognized and accepted upon admission to Ouachita Hills Academy, are credited towards a Ouachita Hills Academy diploma; however, the grades are not used in calculating a cumulative GPA.

 

TRANSCRIPT REQUEST FORM TO OHA:  A transcript fee will be assessed per each transcript request.

5)  COMPLETE STANDARDIZED TESTING & SUBMIT SCORES 

Home schooled students and those coming from non- accredited schools must submit recent standardized test scores (i.e. Iowa Test of Basic Skills, the SAT10, California Achievement Test, etc.). Students coming from accredited schools may omit this step with submission of an official school transcript.

6)  ATTACH A PORTRAIT PHOTO TO THE APPLICATION FORM


7)  PAY THE $10 APPLICATION PROCESSING FEE:

Pay application fee along with application submission.  You may pay online or call the business office to pay the fee.
